Dear Patient of Erindale Health Centre:

I am retiring from my clinic practice on June 1st, 2025. It has been my privilege to have a long career of over 4 decades in family medicine and to have been your family physician.

Unfortunately, finding a replacement family physician to assume my practice has proven impossible to date. Even overseas, I learned that family physicians are in high demand. I will continue to try to recruit on your behalf to find a family physician to assume your care. Erindale Health Centre owners are also recruiting family physicians.

Please search for a new family physician or clinical nurse practitioner. If you fine a new primary health care provider, please sign a transfer of records form at the clinic to have your records transferred. A copy of your file will remain at Erindale Health Centre. The clinic may charge an administrative fee to facilitate the transfer of your records.

If you cannot find a family physician, you can present to the Walk-In Physician at Erindale Health Centre in the interim. The other physicians at this clinic have full practices and are not able to assume the care of my patients. Dr. K Jewitt did indicate that she will take care of my Prenatal patients, newborn babies, and toddlers up until the age of 2y.

I encourage you to contact E-Health and sign up for mysaskhealthrecord so you can see your results online. If you have concerns about results you should consult with a walk-in physician. Please bring in your blood pressure readings and all of your medications and supplements including dosages to all visits. You can get a list from your pharmacy.

Once you have confirmed your email and signed in, you need to go to “My Services” and then choose “Register and view your MySaskHealthRecord”. Then choose “Use Service”.

To sign up for a MySaskHealthRecord account, you will need a valid Saskatchewan health card and your SGI Driver’s license or photo ID handy.
